Requirements for the Renunciation of Philippine Citizenship

For RA9225 applicants (Dual Citizen)

To submit the following :
1. Duly accomplished Affidavit of Renunciation of Philippine Citizenship - one original and two copies
2. Valid Photo ID - three copies
3. Birth Certificate - three copies
4. Original Dual Citizenship documents (Oath of Allegiance, Order of Approval and Identification Certificate) original set and three copies
5. Original Philippine Passport (if available) - one original and three copies
6. Self-addressed pre-paid Express Post envelope with tracking number
 
For non RA9225 applicants
Please submit item nos. 1, 2, 3, 5 and 6.
